• Authorization to Administer Medication:

All prescription and over-the-counter medications, administered to students at school, require an "Authorization to Administer Medication" form signed by the parent and the child's physician.

Medication Authorization forms can be found on the Arbiter landing page

Upload completed forms to Arbiter or return signed copies to the Health Office along with unexpired properly labeled medication.  Prescription medication must be delivered to school in a pharmacy-labeled container which includes the child's name, the medication name, the dose or amount of the medication and the time or frequency, and instructions for use.  For "as needed" medications, the specific symptoms for administering the medication should be included. Nonprescription or over-the-counter medications must be delivered in their original container with the "Authorization to Administer Medication" form signed by the parent and the child's physician.

For questions or assistance, contact school nurse Kelly Schmidt at kschmidt@rossbears.org, or by phone at 415-457-2705, ext. 204.
