• Authorization to Administer Medication:

     All prescription and over-the-counter medications, administered to students at school, require an "Authorization to Administer Medication" form signed by the parent and the child's physician.

     Policy and procedures for completing the "Authorization to Administer Medication" form. 

     

    Return the completed "Authorization to Administer Medication" along with the medication to the school nurse.  Prescription medication must be delivered to school in a pharmacy labeled container which includes the child's name, the medication name,the dose or amount of the medication and the time or frequency, and instructions for use.  For "as needed" medications, the specific symptoms for administering the medication should be included. Nonprescription or over-the-counter medications must be delivered in their original container with the "Authorization to Administer Medication" form signed by the parent and the child's physician.
